PearsonRavitz provides insurance brokerage and consulting services with a focus on disability and life insurance. PearsonRavitz recognizes the importance of maintaining your privacy, and as a result, has established this Privacy Policy (“Privacy Policy”). Throughout this Privacy Policy, the terms “we,” “us,” and “our” refer to PearsonRavitz and the terms “you” and “your” refer to any visitor to or user of the Website or any user of the Secure Intake Form.

This Privacy Policy is intended to help you understand the types of information we may collect from you or that you may provide when you visit the Website, when you access the Portal (as defined herein), or when you submit the secure intake form that we send to you via email to begin your engagement with us (“Secure Intake Form”). Collection of Information

Through the Website, the Portal, email correspondence with you, or through the Secure Intake Form, we may collect “Personally Identifiable Information,” including, but not limited to, your contact information (i.e. name, e-mail address, postal address, telephone number), government-issued identification numbers (i.e. social security numbers or driver’s license numbers), as well as other information such as individually identifiable health information, including but not limited to your demographic data and medical history (“Protected Health Information”), other medical information, financial information, and employment information, subject to the terms of this Privacy Policy, so that we can: (i) provide you with a link to create username and password and access an online portal, which allows you to upload and manage documentation and forms used in connection with your application for certain insurance coverage (the “Portal”); (ii) contact you to respond to inquiries and messages you send to us related to the Website and/or related to purchasing products and services offered by or through us; (iii) review, consider, and/or process documentation or forms submitted to us via the Secure Intake Form or through the Portal; and (iv) communicate with you to provide you with relevant information about PearsonRavitz and the services it offers, including but not limited to, pricing, special promotions and discounts. In all of these cases, we will collect Personally Identifiable Information and Protected Health Information from you only if you voluntarily submit such information to us, and solely for the purposes of providing insurance consulting and brokerage services and other services that we may offer on or through the Website from time to time. However, we may collect non-personal information in order to operate, maintain, and improve the Website and our services.

Protected Health Information

We protect your Protected Health Information as required by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). We will not disclose Protected Health Information that you provide via the Website, the Portal, via email, or via the Secure Intake Form unless one or more of the following conditions are met:

  • We receive your prior written authorization. Answers to questions about your medical history and physical condition are required by our insurance carrier partners and will be released to the insurer so that they may underwrite your insurance application. We will not give or sell such information about you to any other company, individual, or group without your prior authorization;
  • We verify that the prospective recipient is you or your authorized representative
  • We are required by law to disclose the information; and
  • The disclosure is permitted under HIPAA.

The Website and the Secure Intake Form Are Not Intended For Use By Individuals Under 18

No one under the age of 18 may provide any personal information on or through the Website, through the Portal, through the Secure Intake Form, via email or otherwise. We do not knowingly collect personal information, including Personally Identifiable Information, from anyone under 18. If you are under 18, do not: (i) use or provide any information to us via email, through the Secure Intake Form, through the Portal, on the Website or, on or through any of the features available on the Website; (ii) use any interactive features on the Website; or (iii) provide any information about yourself to us, including your name, address, telephone number, e-mail address or any screen name or user name you may use. Security of Information

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of your information, including your Personally Identifiable Information and Protected Health Information, we have put in place reasonable physical, electronic, and managerial procedures to safeguard and secure the information we collect online, and we make good faith efforts to store your information in a secure operating environment.

You must also seek to protect against unauthorized access to any information that you use in connection with the Website or that you submit to us via email, the Portal, or the Secure Intake Form, and you should remember to close the browser when you step away from your computer and when you have completed your activities on the Website. If you choose to create an account on the Website or the Portal (an “Account”), you must treat your user name, password or any other piece of information related to your Account (“Account Information”) as confidential, and except as otherwise stated herein, you must not disclose Account Information to any other person or entity.

You also agree to ensure that you exit from your Account at the end of each session. Use and Sharing of Information

PearsonRavitz facilitates the sale of insurance products offered by third party insurance carriers, and if you request to purchase a policy offered by a third party insurance carrier, we will share the information you submit to us with the insurance carrier, solely in connection with providing the requested products and services.

Except as provided in this Privacy Policy, or as otherwise permitted or required under law, we reasonably attempt to ensure that we do not intentionally disclose any Personally Identifiable Information or Protected Health Information submitted by you through the Website to any third party without having first received your permission (through your opting in or otherwise agreeing to the disclosure of such information).

We will release information, which may include Personally Identifiable Information and/or Protected Health Information, as required by law to comply with any valid legal inquiry or process such as a search warrant, subpoena, statute or court order. We may also release specific information, which may include Personally Identifiable Information, in special cases, such as in certain instances if there is a physical or property threat to you or others. Electronic Communications and Opting in and Opting Out

When you communicate with us electronically, via e-mail or otherwise, you consent to our use of the information you have provided, and you further consent to receive electronic communications from us. Cookies and Other Technologies

The Website may use “cookies” and/or other technologies or files (collectively, “cookies”) to identify how visitors make use of the Website.
